Before You Arrive

WHAT TO BRING:

When you come to your appointment please bring your insurance card and an official Identification card like a Driver's License. If you are not able to complete new patient paperwork beforehand, please arrive 30 minutes early to complete this paperwork which helps us best care for you. Copays will be collected at the time of your appointment.

We are located behind the One Blood Blood Bank on Parental Home Road, just North of Beach Boulevard. The intersection of Parental Home Road and Beach is where the Hart expressway ends at Beach Boulevard. Nearest major crossroads are Southside Boulevard and Beach Boulevard. We are about 1 mile due West from that intersection.
